UPDATE - Deadline Extended to April 29 3pm for Flood Damage Assessments

Pursuant to a request from the Cook County Department of Homeland Security and Emergency Management (CCDHSEM), the City of Park Ridge needs flood damage assessments from residents and businesses in the City. Flood damage information will be collected by the City and forwarded to Cook County. Once the County receives this data, the County will determine if the federal flood damage threshold of $17,921,678 has been reached countywide. This is only the initial collection of flood damage data; no federal or state funds are available at this time.

The City is asking residents and businesses suffering damage resulting from the Flood of April 18 – 19, to complete the Individual Flood Damage Assessment Survey. The survey is available beginning Tuesday morning, April 23 on the City website at www.parkridge.us  left side, in red as Flood Assessment Survey    or click the link to go directly to the survey.

Residents who need assistance in completing the on-line Flood Damage Assessment Survey may contact 847-318-5200, and you will be assisted in completing the survey on-line.

Residents, who do not have computer or Internet access, may complete the Flood Damage Assessment in hard copy. Citizen’s Emergency Response Team (CERT) members will be going door to door in the most affected areas of the City with Damage Assessment Survey forms and will assist you in completing the forms. Further, Damage Assessment Survey Forms will be available at City Hall in the City Manager’s Office located on the first floor.

The deadline for completing the Damage Assessment Survey has been extended to Monday, April 29 at 3 pm.
